Abstract
Introduction: Food allergies and food intolerances are two of the major challenges facing modern medicine. Accurately measuring the prevalence of these conditions remains a considerable challenge. Scientific research has demonstrated a consistent increase in the prevalence of food allergies over recent decades, a trend associated with changes in dietary habits, alterations in intestinal microbiota, the hygiene hypothesis, as well as environmental and genetic factors. Despite this growing prevalence, the Albanian population remains insufficiently studied from a scientific and epidemiological standpoint with regard to these phenomena.
Objective: This study aims to assess, for the first time, the prevalence of food allergies and food intolerances in the Albanian population, thereby contributing to the establishment of a national database essential for the development of health policies, clinical guidelines, and preventive strategies.
Materials and Methods: This cross-sectional study was conducted over the period 2023–2026 and included a large sample drawn from the Albanian population across all age groups, from early childhood through older adulthood. To identify and assess immunologically mediated food allergies, the Polycheck test was applied . A total of 682 individuals underwent this test. For the assessment of food intolerances, the Food Explorer test was employed, an immunological method measuring specific IgG antibodies against 286 distinct food proteins, positioning this panel among the most comprehensive currently available in the field of food intolerance diagnostics. This test enables the identification of delayed food reactions — which frequently remain undiagnosed due to their subclinical and non-immediate nature — thereby providing a comprehensive overview of immunological tolerance to various foods. A total of 92 individuals underwent Food Explorer testing, selected on the basis of clinical symptomatology suggestive of food intolerance.
Study Results: Laboratory data were processed using SPSS statistical software, version
21. In accordance with the study objectives, the principal findings are as follows:
Overall Sensitization Profile: The highest prevalence of sensitization was recorded for fruits, vegetables, and meat — a pattern consistent with European and Mediterranean epidemiological data. These findings indicate a predominance of sensitization to plant-derived proteins.
Sensitization by Age Group: In early childhood, food allergies are predominantly triggered by animal-derived proteins. In adolescence, fruits and tree nuts are the leading sensitizing agents, whereas in adulthood, sensitization to seafood products is most prevalent. This reflects a progressive shift in the sensitization profile across the lifespan.
Sensitization by Sex: Males exhibited more frequent allergic sensitization to food across the majority of categories; however, statistically significant differences were identified only for selected food groups.
Seasonal Variation in Sensitization: Elevated levels of sensitization to vegetables and fruits were recorded during spring, persisting throughout the summer season. In autumn and winter, sensitization to fruits and vegetables declined, while sensitization to animal-derived food products increased. These findings confirm that food sensitization is not a static phenomenon — it is dynamic in nature, influenced by environmental exposure, pollen seasonality, and pollen–food immunological cross-reactivity (Pollen-Food Allergy Syndrome, PFAS).
Regarding food intolerance as assessed by the Food Explorer test, laboratory results revealed high levels of reactivity to egg — specifically egg white at 62% and egg yolk at 59.8% — followed by exotic fruits including pineapple, kiwi, and fig, as well as tomato and wheat (28.3%).
Conclusion: This study is the first in Albania to provide a comprehensive overview of the level of sensitization to different food types and to contribute to the assessment of food intolerance through the use of laboratory testing. It is of particular epidemiological importance, serving as a foundation for future multidisciplinary research in the fields of allergology, immunology, and nutritional medicine.
